The screenshot in that documentation shows an older, pre-Outlook2CRM, product called the Outlook Ticket Addin. The Outlook2CRM product evoloved from that older product. One of the mail differences in Outlook2CRM was that the buttons were dynamically created at runtime based on what the administrator defines in the Toolbar Editor. The Outlook2CRM toolbar can be customized and new buttons added to it. Because of this dynamic loading and Outlook's inability to maintain the state of the toolbar, the toolbar was getting created *every time* the E-mail window in Outlook opened causing a huge performace loss. Because of this the Outlook2CRM toollbar dropped support for having the buttons appear in the E-mail window, as shown in the screenshots you mention, to avoid this performance hit.
This feature *will* be coming back in a later Outlook2CRM release (actual version not yet determined).
